Also, as this was not mentioned. When palpating and attempting to blanch the area, note the feel of the area. Stage 1 pressure ulcers feel hollow or "squishy" as one of my coworkers described it :) This whole thing is easily avoided by moving Pt every 2 hrs and doing thorough integumentary assessments.

I got 'a talkin' to' from my boss because I documented a blister (hospital acquired) on a patient's heel as a stage 2 pressure ulcer. Now I have to do a presentation on staging. Everything I'm reading/watching would call it a pressure ulcer and I am so annoyed!

My mother has a ulcer just like this , and im so scared its not gonna heal . shes now in hospice house . and they are not doing anything about it . they just keep momma on her side .
